How to Design a Connect Card First-Time Guests Fill Out

Most connect cards ask too many questions and get ignored. Here's how to design one that guests actually fill out.

Quick Answer: A connect card gets filled out when it asks for less, not more. Keep it to five fields or fewer, put the most important question first, and give guests a reason tied to something they actually want — not just a form to fill out because you asked.

Most connect cards fail for one simple reason — they read like an intake form instead of a quick note. Full address, phone number, email, "how did you hear about us," a list of ministry interests to check — by the third question, most first-time guests have already set the card down.

A connect card isn't trying to gather everything you might ever want to know about someone. It's trying to capture just enough to follow up well.

What to Actually Include

1. Name and one way to reach them

Name, plus either a phone number or email — not both required. Making both mandatory is one of the fastest ways to lose a hesitant guest halfway through.

2. A simple "first time here?" checkbox

This single checkbox does more work than almost anything else on the card. It lets your follow-up team instantly separate first-time guests from returning members, so the right person gets the right kind of follow-up.

3. One specific next step, not a general interest list

Instead of a long list of ministries to check, ask one focused question tied to what's actually happening soon — "Interested in [upcoming event]?" A specific yes/no gets answered. A long checklist gets skipped.

4. Space for a prayer request (optional, but valuable)

A short open line for a prayer request often gets more genuine engagement than any demographic question you could ask — and it gives your pastoral team a real, meaningful reason to reach out.

Design Details That Actually Matter

Keep the card to a size that fits comfortably in a hand, a Bible, or a seat-back pocket. Our Connect Cards are 3.5" x 8.5", tall and slim, and printed on uncoated 16pt cardstock so pens write cleanly. Use enough white space that it doesn't feel like a form, and make sure the "first time here?" checkbox is visually easy to spot, not buried at the bottom.

Once the cards are filled out, the next step is what you do with them. We shared a few ideas in Five Ways To Use Connection Cards Effectively in Your Church.

Frequently Asked Questions

How many fields should a church connect card have?

Five or fewer works best. Every additional field reduces the odds a first-time guest finishes filling it out.

Should a connect card require both phone number and email?

No — requiring both tends to reduce completion rates. Offering either option, with neither marked as mandatory, generally gets better response.

What's the most important field on a connect card?

A simple "first time here?" checkbox. It lets your follow-up team quickly separate first-time guests from returning members and respond appropriately to each.
